Senate approves $60.4 billion Sandy aid bill

(AP) ? The Senate on Friday approved a $60.4 billion emergency spending aid package for victims of Hurricane Sandy that had been backed by Senate Democrats.

Democrats had to turn back Republican efforts to cut programs such as $150 million in fisheries aid that Republican lawmakers said was unrelated to the storm that hammered the East Coast late in October. The measure cleared the Senate on a 62-32 vote, with 12 Republicans supporting the bill. Sen. Mark Pryor, D-Ark., was the only Democrat to vote against the bill, but he later switched his vote to support the measure.

The bill faces uncertain prospects in the House, where GOP leaders appear reluctant to move quickly on a big spending bill in the final days of a lame duck session. Congress' attention is focused on talks over the so-called fiscal cliff of tax hikes and automatic spending cuts.

Sandy was blamed for at least 120 deaths and battered coastline areas from North Carolina to Maine. New York, New Jersey and Connecticut were the hardest hit states and suffered high winds, flooding and storm surges. Sandy damaged or destroyed more than 72,000 homes and businesses in New Jersey. In New York, 305,000 housing units were damaged or destroyed and more than 265,000 businesses were affected.

Senate Republicans failed on an amendment for a smaller package of about $24 billion in aid for Sandy, which was the most costly natural disaster since Hurricane Katrina in 2005 and one of the worst storms ever in the Northeast.

House GOP leaders have not said how they plan to proceed. But House Appropriations Committee Chairman Hal Rogers of Kentucky has said Congress should probably begin with a smaller aid package for immediate recovery needs and wait until more data can be collected about storm damage before approving additional money next year.

Rep. Paul Ryan, the 2012 GOP vice presidential nominee and a leading House fiscal conservative, has criticized the Democratic bill as "packed with funding for unrelated items, such as commercial fisheries in American Samoa and roof repair of museums in Washington, D.C."

Sen. Charles Schumer, D-N.Y., urged House leaders to "put this bill on the floor quickly and allow a vote." If the House balks, Schumer said, the Senate bill provides "very good groundwork" for seeking Sandy aid next year.

The measure includes $11.5 billion for the Federal Emergency Management Agency's chief disaster relief fund and $17 billion for community development block grants, much of which would help homeowners repair or replace their homes. Another $11.7 billion would help repair New York City's subways and other mass transit damage and protect them from future storms. Some $9.7 billion would go toward the government's flood insurance program. The Army Corps of Engineers would receive $5.3 billion to mitigate flood future risks and rebuild damaged projects.

Senate Republicans said much of the spending in the Democratic bill was for projects unrelated to Sandy, such as $150 million for fisheries disasters that could go to Alaska as well as Gulf Coast and New England states. Sen. Tom Coburn, R-Okla., sought to strip the fisheries funding, but his amendment failed.

To court votes, Democrats last week broadened some of their bill's provisions to cover damage from Hurricane Isaac, which struck the Gulf Coast earlier this year. A provision was added to the $2.9 billion allotted to Army Corps of Engineers projects to reduce future flooding risks; the coverage area for that program will now include areas hit by Isaac in addition to Sandy. Democrats also shifted $400 million into a community development program for regions suffering disasters, beyond areas struck by Sandy.

A Coburn amendment to reduce the federal share of costs for the Army Corps of Engineer projects to reduce future flooding risks also failed.

Most of the money in the $60.4 billion bill ? $47.4 billion ? is for immediate help for victims and other recovery and rebuilding efforts. The aid is intended to help states rebuild public infrastructure like roads and tunnels, and help thousands of people displaced from their homes.

"It will actually put people to work in their own communities, rebuilding their own communities," said Sen. Barbara Mikulski, D-Md., who chairs the Senate Appropriations Committee.

GOP Sens. John McCain of Arizona and Coburn, two frequent critics of government spending, targeted what they called "questionable" spending in the Democratic bill, including $2 million for roof repairs at Smithsonian Institution museums and $58 million in subsidies for tree planting on private properties. A McCain amendment to strip the tree subsidies failed.

Republicans also criticized $13 billion in the Democratic bill for projects to protect against future storms, including fortifying mass transit systems in the Northeast. Republicans said however worthy such projects may be, they are not urgently needed and should be considered by Congress in the usual appropriations process next year.

The Congressional Budget Office has estimated that only about $9 billion of the $60.4 billion proposed by Democrats would be spent over the next nine months. The Democratic bill included many large infrastructure projects that often require years to complete, but Republicans said the CBO estimate of such drawn-out spending undercuts the urgency of the Democrats' aid package.

More than $2 billion in federal funds has been spent so far on relief efforts for 11 states and the District of Columbia. FEMA's disaster relief fund still has about $4.3 billion, and officials have said that is enough to pay for recovery efforts into early spring.

New York, New Jersey, Connecticut, District of Columbia, West Virginia, Virginia, Maryland, New Hampshire, Delaware, Rhode Island, Pennsylvania and Massachusetts are receiving federal aid.

    Is air marshal program a model for schools?

    (AP) ? The former congressman who's heading the National Rifle Association's school emergency response program that would include volunteers to help guard children says it makes sense because it's similar to placing air marshals on planes.

    Asa Hutchinson tells ABC's "This Week" that the air marshal program has provided a deterrent and made flying safer.

    He says putting trained guards such as retired police officers or military persons at schools will help protect students. Hutchinson says hiring guards to defend schools is a "very reasonable approach" but it should be a local choice.

    New Jersey Gov. Chris Christie, a prominent Republican, says the approach will turn schools into an armed camp for kids.

    Food Review: Capstone- Deluxe High Tea and Chocolate Fondue ...

    One of my latest food adventures is due to The Red Pocket which had a coupon I bought a month ago- $38 for Deluxe High Tea + Chocolate Fondue for Two at Capstone Richmond. The coupon included

    There are two locations, and the coupon was only valid for Capstone Tea and Fondue in Richmond. It is located on Capstan Way, so commuters may have a harder time arriving there, but there?s lots of parking space for drivers!

    Reservations had to be made, and luckily we weren?t in trouble for being 20 minutes late. All we had to do when we arrived was order the drinks, as everything else was already pre-pared. My friend ordered a matcha frappe, and I ordered a Fresh Fruit Sparkling Tea- Blueberry Fruit Tea.

    I was extremely thirsty, and this drink quenched it! It was so refreshing, and it wasn?t sweet. The sparkling bit was delicious and the various fruits mixed in made this a very,?necessary?drink for my thirsty self.

    Then the edibles came!

    The pastries on the left are banana bred, sponge cake (fairly dense and yummy,) and matcha cookies. Then for fruit we got bananas, pineapples, green apples, grapes, and strawberries. They all tasted fresh (I skipped on the banana cake and pineapple though, not my favourite foods) and tasted better dipped in chocolate!

    My favourite part are the ice cream truffles. We only got 6 to share- the flavours were coconut, mango, and raspberry (I think.) The coconut ice cream was a dream!

    Along with fondue, my friend and I each got a high tea set ? a three tier tray of savouries and sweets.

    There were two flavours of sandwiches, ham and cucumber salad. I?preferred?the cucumber salad one better, the ham one tasted a bit too ordinary. The rest were mini croissants that weren?t too outstanding, although they were cute. The tiny chocolate croissant was delicious though.

    I do wish there was a bit more variety- doesn?t high tea need scones? They could?ve taken a croissant/danish or two away.

    This little mini green sweet was delicious because it tasted like a fererro rocher- hazelnut and chocolate is the best combination ever!

    If you have been following my blog, you know I love my macarons. I was surprised to find that Capstone had fairly decent macarons, albeit the taste was a bit artificial. However I liked the soft, chewy texture and it was decent sized as well. The coffee macaron was divine!

    I prefer the ice cream fondue over the high tea set for sure. Their ice cream truffles never fail to disappoint me. If you?re interested in trying the afternoon tea set, it is $14.95 + includes a drink!

    Overall, I enjoyed my meal at Capstone. There was a lot of food, and I couldn?t finish it all. They do not have boxes for carry out, but the server kindly gave me plastic cups (for to-go drinks) to put my extra croissants away. Additionally, the service was great! Perhaps there weren?t a lot of people when we went on a Thursday night, but the two people were working there were very attentive and came to ask if we were enjoying everything. It was also nice and quiet so my friend and I enjoyed our meal in peace.

    India Promises Defense Aid to Somalia

    The Indian High Commissioner in Nairobi, Sibabrata Tripathi, has expressed the Indian government's readiness to actively support Somalia's effort to rebuild its defense forces with a view to enhance overall regional security.

    His statement follows an official visit he made to the Somali Ministry of Defense in Mogadishu on Monday (December 17th) where he held talks with the Somali Defense Minister, Abdihakim Haji Mohamud Fiqi, on a wide range of ongoing issues. During the discussion, the visiting High Commissioner praised recent positive developments in Somalia and expressed his country's readiness to provide military aid to help bolster Somalia's security forces.

    Mr. Fiqi, who noted the recent positive gains against Al-Shabaab with the help of AMISOM and other positive forces, said terrorism was still the biggest threat to Somalia. He highlighted the new government's ongoing security commitments against terrorism, piracy and other security challenges.

    The visit of the Indian High Commissioner was the first visit by an Indian official to Somalia since the collapse of the Somali central government in 1991, but India is, reportedly, the second country after Turkey to promise direct military assistance to the new Government.     Moments of silence around NFL for shooting victims

    FOXBOROUGH, Mass. (AP) ? The New England Patriots silenced their "End Zone Militia" on Sunday night, taking the muskets away from the Revolutionary War re-enactors who fire into the air to celebrate every score.

    The memory of the Connecticut school shooting was still too fresh for the sight of firearms and the smell of gunpowder.

    "It just doesn't show the right respect for those that lost their lives," said Bob Elliott, the group's sergeant. "But we're still here cheering (the Patriots) on."

    Two days after 20 children and six adults were shot to death at the Sandy Hook Elementary School in Newtown, Conn., NFL fans gathering in stadiums across the country honored the victims' memory with periods of silence and reflection. Some teams darkened their scoreboards and lowered their flags to half-staff, while others wore helmet decals or black ribbons.

    After learning he was the favorite player of one 6-year-old victim, New York Giants receiver Victor Cruz wrote "R.I.P. Jack Pinto," ''Jack Pinto, my hero" and "This one is for you" on his shoes for the game against the Falcons in Atlanta. Cruz said he called the boy's family after hearing he was a Giants fan and was told they planned to bury him in one of Cruz's No. 80 jerseys.

    "I don't even know how to put it into words," Cruz said. "There are no words that can describe the type of feeling that you get when a kid idolizes you so much that unfortunately they want to put him in the casket with your jersey on. I can't even explain it."

    The Patriots, the closest team to Newtown that played at home on Sunday, wore a helmet sticker with the city seal and a black ribbon on it; the cheerleaders and mascot wore black armbands, and owner Bob Kraft pledged $25,000 to the community, where he also owns a box-making factory. Before the game, the public address announcer asked for silence while 26 flares were sent into the air.

    But each time the Patriots scored in the 41-31 loss to San Francisco, the soldiers in the End Zone Militia clapped their empty hands like the rest of the crowd. Elliott said the Patriots asked the group, which has been standing sentry at home games since the mid-1990s, to skip the ceremonial fire.

    "Out of respect for those that were killed, we were asked yesterday not to fire the muskets, which we all agreed with," said Elliott, who is a manufacturing manager for a dental implant-maker. "It was just such a horrific thing. It's hard to put it into words."

    The Sunday Night Football broadcast on NBC was moved to CNBC and the NBC Sports Network while President Barack Obama addressed the nation. The game returned to its regular channel after the president's remarks from Newtown.

    The Giants, another popular team in southwestern Connecticut, affixed a decal with the school's initials ? "SHES" ? on their helmets.

    "Being close to home, the players were greatly upset about it," Giants coach Tom Coughlin said. "Many of the players have young children so they can empathize with the parents who had young children killed."

    Houston Texans defensive end J.J. Watt wrote "Newtown, CT" on one of the gloves he wore in warmups and on both of his shoes for the game.

    "We're playing football, and there's something much bigger going on in this world," Watt said. "I just wanted them to know, and I wanted everyone to know, that our thoughts are with them. Nothing is bigger than that. We played our game today, but honestly our thoughts are with them, the families, the teachers, the friends, the first responders, who had to go see that. My dad is a first responder. They were just kids."

    In St. Louis, the players who wear No. 26 ? Rams running back Daryl Richardson and Vikings cornerback Antoine Winfield ? joined hands in a circle with their coaches at midfield before their game, surrounded by dozens of children wearing jerseys.

    "I have a son that's in kindergarten. It choked me up because I would hate to be one of those parents," Rams running back Steven Jackson said. "You drop your kid off at school and he or she wants to go there and learn and better themselves, and to then go to the school and find that your child will no longer be with you. I couldn't imagine that thing."

    Flags were also at half-staff in Baltimore, where the scoreboards went black as the public address announcer asked the crowd at the game between the Ravens and Denver Broncos to observe "silent reflection" in the wake of Friday's "horrific tragedy."

    "As a parent you drop your kids off at school many times," said Miami Dolphins coach Joe Philbin, whose 21-year-old son Michael fell into a Wisconsin river and drowned in January. "It's hard to put into words what that community and those families must feel like. We obviously kept them in our prayers."

    A moment of silence was observed at all 14 NFL games on Sunday; in Houston and in Arlington, Texas, the scoreboard went black. Members of the Buffalo Bills and Seattle Seahawks stood quietly with their heads down on their sideline while fans stood silently at the Rogers Centre in Toronto.

    The Bills did continue their pregame habit of playing U2's "Sunday Bloody Sunday," which they've played before every home game this season. The song is in reference to British troops shooting and killing unarmed protesters in Derry, Northern Ireland in January 1972.

    In Chicago, Green Bay wide receiver Donald Driver retweeted the names of the victims. St. Louis defensive end Chris Long said after the 36-22 loss the Vikings that it was hard to feel sorry for himself.

    "As we sit here and feel sorry for ourselves after losing a football game, it really helps put things in perspective," he said. "I was watching TV last night and saw a victim's parent and I was really moved by that, the strength that they were showing up there. If we can all show that strength, we'll be all right as a team and as people."

    All a question of timing: When bushcrickets mate, the male attaches a sticky package, the so-called spermatophore, to the female's abdomen. Alongside the sperm themselves, this 'bridal present' consists of a protein-rich mass that the female eats after mating. It then takes several hours for the sperm to find their way into the female's reproductive tract. But, who decides when that will happen? A study by the Bielefeld biologists Professor Dr. Klaus Reinhold and Dr. Steven Ramm suggests that it is the male who determines the dynamics of this process even when he has long 'hopped off' somewhere else. They have now published their results in the online first version of the journal Behavioural Ecology and Sociobiology.

    In contrast to direct sperm transfer, the use of a spermatophore could grant the female more influence over the fertilization or non-fertilization of her eggs. However, the results of the Bielefeld study cast doubt on this assumption. They suggest a high degree of male control over this decisive stage in reproduction. For their study, Professor Dr. Klaus Reinhold and Dr. Steven Ramm from Bielefeld University paired males and females from two subspecies of the bushcricket Poecilimon veluchianus in whom the time between pairing and sperm transfer differ. Whereas in the subspecies Poecilimon veluchianus minor, sperm are transferred within the first three hours, the transfer in Poecilimon veluchianus veluchianus starts only after four hours. If the two subspecies are interbred the researchers thought then the number of transferred sperm after three hours would indicate whether it is the male or the female who determines how long this transfer takes.

    The researchers mated nine to twelve pairs in each of the four possible combinations of Poecilimon veluchianus minor and Poecilimon veluchianus veluchianus. Three hours after mating, they examined how many sperm they could find in the female's reproductive tract. The result: the sperm from the males of the 'faster' subspecies Poecilimon veluchianus minor could be found in the females of both subspecies. In contrast, the males in the 'slower' sub-species Poecilimon veluchianus veluchianus had transferred almost no sperm at all to either type of female.

    The researchers conclude from this experiment that the males control the speed of transfer over the sperm package. However, this does not automatically lead to the conclusion that the female is powerless. Females can also influence whether sperms are transferred by how quickly they eat the spermatophore. In addition, the larger the male, the larger the size of the sperm package, and this influences how long the females need to consume the protein. As a result, the sperm have more time to transfer to the female and the female's eggs have a greater chance of being fertilized by a 'high-quality' male. Professor Reinhold stresses, 'Our findings show that the females do not determine the transfer not that they could not do so.'

    UMHS approach to medical errors and malpractice suits could be used by hospitals nationwide, new study indicates

    ANN ARBOR, Mich. The University of Michigan Health System doesn't claim to be perfect. But its response to medical errors, near-misses, unexpected clinical problems and unintended outcomes is a model for the nation that other hospitals can and should copy, according to a new paper in a prestigious health care journal.

    The "Michigan Model" for handling these situations, and preventing them from happening again, has not only helped patients and medical staff alike it has also helped UMHS go against the grain of the costly, combative "deny and defend" medical malpractice culture.

    In a paper in December issue of the Milbank Quarterly, and in a presentation today at a meeting of the U-M Board of Regents, the UMHS approach is once again in the spotlight for its potential to be emulated by hospitals across the country.

    The new paper, authored mainly by Massachusetts-based researchers, lays out the fundamentals of the model for other hospitals to emulate. The authors, from Harvard Medical School and the Massachusetts Medical Society, report that key stakeholders across the medical and legal community see the Michigan approach as a feasible and promising approach for their state.

    The presentation to the U-M Regents, given by UMHS chief medical officer Darrell A. Campbell, Jr, M.D. and executive director of clinical safety Rick Boothman, J.D., lays out further the details and results of the UMHS approach. They also featured video clips from actual patients and patient family members, who have told their unvarnished stories for a video aimed at every member of the UMHS care team.

    "By handling unanticipated and unintended incidents, and patient injuries, honestly and proactively, we've virtually eliminated groundless legal claims, allowing us to focus on issues that demand attention with clear vision and no more excuses," says Boothman. "We fundamentally focus on putting patients and safety first, and we believe other hospitals can do the same."

    Campbell and Boothman have led a decade-long effort to implement and measure the results of the Michigan Model. It's based on these key principles:

    • Compensate patients quickly and fairly when inappropriate care causes injury
    • Support clinical staff when the care was reasonable
    • Reduce patient injuries (and claims) by learning from patients' experiences

    In that decade, new malpractice claims per month have dropped, total liability costs have dropped, claims and potential claims are being resolved faster, and UMHS is increasingly avoiding litigation in both claims without merit and claims with merit.

    The authors of the new paper, who interviewed 37 physicians, hospital executives, attorneys, public policymakers, insurers and others across Massachusetts, find a general consensus that the Michigan approach also called DA&O for "disclose, apologize and offer" -- holds great potential to improve medical liability and patient safety.

    They write: "It was viewed as more promising than any other liability reform option, both on its merits and because it would not be stymied by political gridlock in state legislatures, as other tort reforms frequently have been." They also note that it offers a "value proposition" to patients that's crucial in this age of federal health care reform.

    The authors also note that their interviews indicate that experts across the board do see challenges in implementing the Michigan Model in other hospitals and health care settings.

    But, they conclude, based on the evidence available, "DA&O programs may prove not only to constrain liability costs but also to improve access to compensation, strengthen linkages between the liability system and patient safety, increase health care organizations' accountability and patient advocacy, and promote transparency in regard to medical error."

    Boothman, who is a co-author of the new paper, notes that the Michigan Model or DA&O has also been put forth as a model by the federal Agency for Health Care Research and Quality, which has issued grants for teams to study implementation of the Michigan Model as the leading response to the malpractice crisis. There have also been legislative and research efforts in Massachusetts, Washington, New York, Illinois, Oregon and Florida based in part on the DA&O approach, and interest from four other countries.

    UMHS approach to medical errors and malpractice suits could be used by hospitals nationwide, new study indicates

    ANN ARBOR, Mich. The University of Michigan Health System doesn't claim to be perfect. But its response to medical errors, near-misses, unexpected clinical problems and unintended outcomes is a model for the nation that other hospitals can and should copy, according to a new paper in a prestigious health care journal.

    The "Michigan Model" for handling these situations, and preventing them from happening again, has not only helped patients and medical staff alike it has also helped UMHS go against the grain of the costly, combative "deny and defend" medical malpractice culture.

    In a paper in December issue of the Milbank Quarterly, and in a presentation today at a meeting of the U-M Board of Regents, the UMHS approach is once again in the spotlight for its potential to be emulated by hospitals across the country.

    The new paper, authored mainly by Massachusetts-based researchers, lays out the fundamentals of the model for other hospitals to emulate. The authors, from Harvard Medical School and the Massachusetts Medical Society, report that key stakeholders across the medical and legal community see the Michigan approach as a feasible and promising approach for their state.

    The presentation to the U-M Regents, given by UMHS chief medical officer Darrell A. Campbell, Jr, M.D. and executive director of clinical safety Rick Boothman, J.D., lays out further the details and results of the UMHS approach. They also featured video clips from actual patients and patient family members, who have told their unvarnished stories for a video aimed at every member of the UMHS care team.

    "By handling unanticipated and unintended incidents, and patient injuries, honestly and proactively, we've virtually eliminated groundless legal claims, allowing us to focus on issues that demand attention with clear vision and no more excuses," says Boothman. "We fundamentally focus on putting patients and safety first, and we believe other hospitals can do the same."

    Campbell and Boothman have led a decade-long effort to implement and measure the results of the Michigan Model. It's based on these key principles:

    • Compensate patients quickly and fairly when inappropriate care causes injury
    • Support clinical staff when the care was reasonable
    • Reduce patient injuries (and claims) by learning from patients' experiences

    In that decade, new malpractice claims per month have dropped, total liability costs have dropped, claims and potential claims are being resolved faster, and UMHS is increasingly avoiding litigation in both claims without merit and claims with merit.

    The authors of the new paper, who interviewed 37 physicians, hospital executives, attorneys, public policymakers, insurers and others across Massachusetts, find a general consensus that the Michigan approach also called DA&O for "disclose, apologize and offer" -- holds great potential to improve medical liability and patient safety.

    They write: "It was viewed as more promising than any other liability reform option, both on its merits and because it would not be stymied by political gridlock in state legislatures, as other tort reforms frequently have been." They also note that it offers a "value proposition" to patients that's crucial in this age of federal health care reform.

    The authors also note that their interviews indicate that experts across the board do see challenges in implementing the Michigan Model in other hospitals and health care settings.

    But, they conclude, based on the evidence available, "DA&O programs may prove not only to constrain liability costs but also to improve access to compensation, strengthen linkages between the liability system and patient safety, increase health care organizations' accountability and patient advocacy, and promote transparency in regard to medical error."

    Boothman, who is a co-author of the new paper, notes that the Michigan Model or DA&O has also been put forth as a model by the federal Agency for Health Care Research and Quality, which has issued grants for teams to study implementation of the Michigan Model as the leading response to the malpractice crisis. There have also been legislative and research efforts in Massachusetts, Washington, New York, Illinois, Oregon and Florida based in part on the DA&O approach, and interest from four other countries.
